Scotland's health minister blamed "an error, not a conspiracy" after draft guidance for care homes was published before it had been officially approved.
Jeane Freeman was accused of "incompetence" by opposition MSPs after the document, which advised on testing and admissions policy, was sent out to industry bosses despite not being signed off by ministers.
It was also published on the Scottish Government website before being removed.
