A prisoner died after he accidentally set himself on fire while trying to smoke drugs in his cell. Kevin Delahunty, who was serving five years for burglary, died in hospital after he was found on fire at HMP Risley in Warrington.
When the 40-year-old, of Bradford, West Yorkshire, was found, he had significant burns to his body and his upper clothing was barely visible as a result of the fire, YorkshireLive reports.
A makeshift pipe was found in his cell which was found to contain psychoactive substances. An investigation by the Prisons Ombudsman found had he had a history of substance misuse and there was evidence he took drugs, including psychoactive substances (PS), at Risley.
He had been found under the influence of PS more than 13 times in less than a year. In a report, the Ombudsman said: "The evidence indicates that before he died, Mr Delahunty tried to smoke PS, which he ignited from the electrical socket in his cell, and that his clothes accidentally caught fire while he was under the influence of the PS." The Ombudsman described the details of the investigation as 'truly shocking'.
