Fellow prisoners have said he was in pain for days beforehand, but claimed his pleas to be taken to hospital were refused.A Significant Adverse Event Report (SAER) by NHS Lothian has now concluded that staff failed to act on concerns raised when Stewart was examined 18 days before his death.It also said he should have been put under closer observation after a nurse saw him on the day he died.The report revealed that Stewart complained of back pain in January 2020, shortly after his arrival at Addiewell where he was serving a sentence for attempted housebreaking.A nurse decided this was a muscular problem and prescribed a painkiller.

He was also told to improve his posture.On March 2, Stewart was told he had tested positive for Hepatitis C.Nine days later a nurse found that he had "significant anaemia",
