Dad found dead in Norwich home with daughters 'missed emergency mental health assessment'

The man found dead at a home with his sister-in-law and two daughters missed an emergency mental health assessment just weeks before the tragedy, it has emerged.Bartlomiej Kuczynski, 45, was discovered dead at a property in Costessey, near Norwich, on Friday morning, alongside his daughters Jasmin, 12, Downreuang, seven, and their aunt, Kanticha Noon, 36.Norfolk Constabulary has referred itself to the Independent Office for Police Conduct (IOPC) after failing to attend a 999 call made by a man inside the home earlier that day.It is understood the IOPC is also investigating contact Norfolk Police had with Mr Kuczynski on December 14 after he was reported missing, the Mirror reports.The father-of-two managed to leave a hospital in Colney, Norfolk, unescorted before an emergency mental health assessment was carried out.Residents in the area said Mr Kuczynski had asked for help and had been suffering from mental health issues for months, but they did not know if he had engaged with mental health services.Last night, Chief Constable of Norfolk Police, Paul Sanford, said the force would be "open and transparent" with its investigation.It is believed Norfolk Police are working to reduce the amount of time officers spend on mental health-related incidents through a phased introduction of a scheme called Right Care, Right Person.
