A five-week old baby died after sleeping in bed next to its parents, a serious case review has revealed. The child, referred to as Baby MD, was found ‘lifeless and unresponsive’ by their mother in their Salford home, the report has said.
She had put the baby next to her in bed after a 3am feed, where she was sleeping with the child’s father. Baby MD’s mother was not supposed to be in contact with the child’s father, who had a history of domestic abuse, the report said.
But the review has suggested that written contracts drawn up by local authorities - which required the child’s mother to block contact with the baby’s father - were ineffective and inappropriate, given the background of domestic abuse.
